This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.## Qualifications**Qualifications:*** Licensed Vocational Nurse, per the state requirements.* 1 year of Home Health experience* Good communication skills, assessment skills, and strong clinical documentation skills.* Ability to read, write, and speak the English language.* Reliable transportation. Valid and current auto liability insurance.**Essential functions:*** Under the direction of the RN, assist in identifying the patient’s physical, psychosocial, and environmental needs.* Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.* Provide care utilizing infection control measures that protect both staff and patient (OSHA).* Records treatments administered in medical record.* Records clinical documentation according to accepted regulatory, corporate and professional guidelines.* Participates in weekly case conferences.* Participates in interdisciplinary communication to ensure that patients’ needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.* Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.* Participates in in-service training programs.* Responsible for assuring patient safety.* Complies with state regulations and national standards* All documentation will be completed and synched by 9:00AM the following day, 7 days per week.***We are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.****Please visit*
