Sutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner to join our Cardiology team at 1580 Valencia Street in San Francisco, California. This role supports two cardiologists in a growing outpatient practice and is essential to improving patient access and care continuity.

About the Opportunity
The NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases. The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure. A key part of this role includes educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications to support long-term health.

Key Responsibilities

Conduct follow-up visits for established cardiology patients

Support transitional care management through post-discharge visits

Provide patient education and promote medication adherence

Collaborate with cardiologists and care teams to ensure continuity of care

Document patient encounters in Epic

Requirements
Education: Graduate of an accredited Nurse Practitioner (NP) program.

Licensure & Certifications:

Current California NP license

DEA Controlled Substance Registration Certificate (CSRC)

Current BLS and ACLS certification

Experience:

Experience in cardiology or internal medicine preferred

New graduates with a strong interest in cardiovascular care are welcome to apply
