Responsibilities
Provide direct social services to patients to restore optimum social and health adjustment.
Assess social and emotional factors to minimize impact on patients’ health and coping with daily living.
Help patients and caregivers understand, accept, and follow medical recommendations.
Assist patients and caregivers with personal and environmental difficulties that affect illness or ability to obtain benefits.
Identify resources, such as caregivers and community agencies, to assist patients in resuming community life including discharge planning.
Assist providers and home‑care team members in understanding significant social and emotional factors related to health problems.
Participate in the development of the plan of care.
Qualifications – All States
Master’s Degree from a school of social work accredited by the Council on Social Work Education.
One year of social‑work experience in a health‑care setting.
Plus: Florida Specific Requirements
Current state license as a Licensed Clinical Social Worker (LCSW), pursuant to Chapter 491, F.S.
