Job Description
Key Responsibilities:
Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance.
Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care.
Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions.
Collect, assess, and evaluate patient health data to develop and implement care plans.
Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.
Utilize EMR and other systems to document care accurately and efficiently.
Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency.
Qualifications
Required Qualifications:
Graduate of an accredited Nursing Program and active NJ RN license
Minimum 2 years of clinical nursing experience
BLS certification
Experience assessing patient concerns in a clinical setting (in-person, phone, or electronic communication)
Ability to evaluate patient symptoms and elevate clinical issues appropriately
Ability to review and identify abnormal or critical laboratory values
Preferred Qualifications:
BSN
Prior experience in endocrinology
Experience with patient follow-up, in-basket management, or outbound patient communication
Familiarity with diabetes and endocrine conditions (e.g., diabetes, thyroid disorders)
Knowledge of diabetes medications, including insulin therapies and common medication names
