Job Description:
Conduct monthly member outreach calls and ensure HIPAA compliance
Perform LTSS and SDOH assessments to evaluate member needs
Coordinate care services including appointments, transportation, and medication support
Document member interactions accurately in care management systems (e.g., DCMS)
Identify and escalate clinical concerns to appropriate care managers
Assist members with care coordination to improve health outcomes and reduce gaps in care.
Bi-lingual skills are strongly preferred in Spanish
