
Inpatient Care Manager (RN)- Full Time, 8 AM - 4 PM, Atlantic Health Morristown

JOB DESCRIPTION
The Inpatient Care Manager plays a critical role in coordinating, monitoring, and evaluating patient care throughout the hospitalization and discharge process. This role collaborates closely with physicians, nurses, social workers, and other healthcare professionals to ensure the delivery of high‑quality, cost‑effective care and the safe transition of patients to the next level of care.
Principal Accountabilities
Care Coordination & Patient Advocacy
Manage, coordinate, and facilitate care delivery through comprehensive patient and family assessments.
Collaborate with multidisciplinary teams to support optimal clinical and psychosocial outcomes.
Utilization & Documentation Oversight
Conduct daily chart and utilization reviews to ensure appropriate level of care.
Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record (EMR).
Interdisciplinary Collaboration
Lead and participate in interdisciplinary care rounds.
Identify and proactively address quality‑of‑care concerns, barriers to progression of care, and discharge delays.
Discharge Planning
Formulate, coordinate, and implement individualized discharge plans.
Ensure safe, timely, and effective transitions across the continuum of care, including post‑acute services and community resources.
Resource & Outcome Management
Monitor healthcare resource utilization and length of stay.
Partner with the Supervisor/Manager of Case Management to evaluate outcomes and improve care delivery processes.
Qualifications
Required Qualifications
Current
RN license
in the State of New Jersey
Bachelor of Science in Nursing (BSN)
required
Minimum
2-3 years of recent acute care nursing experience
Strong clinical assessment, critical thinking, and care coordination skills
Effective communication and collaboration abilities
Experience working with
electronic medical records
(EMR)
Preferred Qualifications
Prior experience in
Care Management, Case Management, Utilization Review, or Discharge Planning
Knowledge of payer requirements, regulatory standards, and care transitions
Case Management certification ( CCM ) or
