Clinical Support Nurse LPN

Midwest Health is looking for a Clinical Support RN serves as a primary clinical responder and escalation resource, providing real-time guidance, clinical decision-making, and care coordination to assist communities and provide support as needed.

This person will work with the Executive Directors and RCCs (Resident Care Coordinator) to do the following:

Perform real-time guidance as part of quality assurance and respond to incoming calls and alerts

Manage and document complex clinical situations (falls, change in condition, behaviors)

Advise on appropriate level of care and recommend for resident interventions

Communicate with physicians, families, and staff

Document interactions in EMR or other software

Review, assess, educate, and provide guidance on senital events and participate in the QAPI program

This person have a compact/multistate Licensed Practical Nurse (LPN) license.
