
RN Case Manager Hospice

Overview
Full‑Time RN Case Manager – days shift in Chester and Montgomery Counties, West Virginia.
Annual salary: $90,000.00 – $93,000.00.

Responsibilities
Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other hospice care disciplines as needed or document the rationale for not doing so.
Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
Record pain/symptom management changes/outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Regularly communicate with clinical manager and care team.
Perform on‑call responsibilities and provide on‑call services to patients/families as assigned.
Other duties as assigned.
Qualifications
One year of nursing experience.
Current RN license specific to the state(s) where assigned.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.
