Job Description
We are seeking a skilled and compassionate Nurse Practitioner (NP)/ Physician Assistant (PA) or Medical Doctor (MD/DO) to support facilities in Merced, California. This role focuses on providing high-quality care to patients in skilled nursing and assisted living facilities, primarily within post-acute and long-term care settings.
Position Details
Part‑time, per diem role (approximately 2–4 shifts per month)
Hybrid model (in-person care + remote patient monitoring)
Flexible scheduling based on clinical needs
Competitive per-patient compensation based on experience and volume

Key Responsibilities
Conduct comprehensive patient assessments and physical examinations
Develop and manage treatment plans in collaboration with supervising physicians
Review and interpret laboratory results and diagnostic data
Provide patient and family education on health management and chronic conditions
Document patient care accurately in electronic medical records (EMR)
Coordinate care with interdisciplinary teams and facility staff
Support ongoing patient monitoring and follow-up care
Qualifications
Active and unrestricted NP or PA license in California
Experience in post-acute, primary care, or related clinical settings preferred; open to candidates with strong clinical training and interest in learning
Strong clinical judgment, communication, and interpersonal skills
Experience with EMR systems and comfort with telehealth or remote patient monitoring (preferred)
Ability to work independently while collaborating effectively with a care team
