What You’ll Do:
Perform in-home patient assessments
Develop and follow individualized care plans
Provide skilled nursing care (wound care, meds, catheter care, chronic disease management)
Educate patients and caregivers on home care and safety
Monitor patient progress and update care plans
Complete timely and accurate documentation
What We’re Looking For:
Active
Texas RN license (required)
Home health or acute care experience preferred
Strong clinical judgment and adaptability
Excellent communication and patient care skills
Reliable transportation & valid driver’s license
Kinnser/WellSky experience (preferred)
Pay & Benefits:
$50–$120 per visit
Flexible, self-managed schedule
