Primary Purpose
Join our expanding team as a certified Nurse Practitioner conducting in‑home wellness risk adjustment assessments for Medicare members and other at‑risk populations, ensuring accurate risk adjustment coding and enhancing value‑based care.
NP Responsibilities
In‑home Clinical Assessments
Travel to members’ homes to conduct comprehensive health evaluations.
Perform medical history, physical examinations, review of systems, vital signs collection, medication review, and possible preventive health screenings.
Conduct annual wellness visits and assessments for chronic‑condition members.
Include procedures such as Hemoglobin A1c, spirometry, kidney health evaluation (KED), diabetic retinal exam (DRE), FIT.
Patient Education & Engagement
Deliver preventive health education and counseling.
Educate patients on medication adherence and chronic condition management.
Support informed decision‑making and consent.
Engage in care discussions when appropriate.
Care Coordination & Quality Support
Identify clinically relevant diagnoses for care management and risk adjustment documentation.
Communicate findings to generate post‑visit summaries for primary care providers.
Identify and close quality care gaps such as screenings, labs, and follow‑ups.
Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience.
Compliance & Professional Standards
Adhere to HIPAA regulations and protect PHI.
Maintain accurate, timely, and compliant EMR documentation.
Perform additional duties as assigned to support organizational goals.
Contribute enthusiastically to enhancing care delivery.
Providers are expected to commit a minimum of 30 hours per month.
Locations & Travel Requirements
This position is based in Shelby County, Tennessee and requires daily travel within a 55‑mile radius of Memphis and surrounding areas.
NP Qualifications
Valid, unencumbered Nurse Practitioner license in the applicable state.
Willingness to travel up to 55 miles from assigned county.
Preference for weekday schedules and standard working hours.
Previous in‑home risk assessment experience preferred.
Prior 1099 (PRN) experience preferred but not required.
3+ years of patient‑care experience (primary care/adult/geriatric, EMR) preferred.
Ability to obtain additional NP licensure if requested.
Reliable transportation to travel to members’ homes within the designated area.
Strong proficiency with EMR systems.
Ability to work independently.
Bilingual abilities are a plus.
