About the Role
The Nurse Navigator is a key member of our care coordination team, supporting moderate- to high-risk patients as they navigate the healthcare system. This role focuses on identifying barriers to care, promoting treatment adherence, and empowering patients to actively participate in their health journey.
Working closely with physicians, clinical teams, patients, and community partners, the Nurse Navigator ensures patients receive the
right care, at the right time, in the right setting . This position is ideal for a nurse who enjoys problem-solving, patient education, and collaboration in a moderately paced, complex care environment.
What You’ll Do
Coordinate care for moderate- to high-risk patients, including scheduling appointments, managing referrals, and addressing barriers such as transportation or medication access
Serve as a trusted liaison between patients, primary care providers, specialists, and support services
Support care plan development and implementation, ensuring patient understanding through techniques such as teach‑back
Review medical records, clinical documentation, and insurance data to assess patient needs and develop individualized navigation plans
Conduct post‑visit and post‑discharge outreach to reinforce care plans and support follow‑through
Educate patients on chronic disease management, navigating the healthcare system, and preparing for appointments
Connect patients with appropriate community resources and support services
Document all patient interactions, care coordination activities, and outcomes accurately in the EMR
Collaborate with fellow nurse navigators and clinical staff to deliver seamless, patient‑centered care
Participate in quality improvement initiatives and staff education related to patient‑centered and chronic care models
Complete additional duties or special projects as assigned
What We’re Looking For
Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Minimum of
3 years of experience
in an outpatient healthcare setting
Strong knowledge of chronic disease management and patient education principles
Excellent communication, organizational, and problem‑solving skills
Proficiency with electronic medical records and basic computer applications
Thorough understanding of HIPAA and commitment to patient confidentiality
Ability to work independently while collaborating effectively within a multidisciplinary team
