Sutter Medical Group is seeking a Post‑Acute & Geriatric Medicine Physician to join our physician‑led post‑acute care team in Sacramento, CA.

This role is ideal for physicians who enjoy caring for medically complex patients during the critical transition from hospital to recovery, and who value autonomy, continuity, and long‑term patient impact.

You will serve as the primary physician of record for patients in skilled nursing and rehabilitation settings, managing complex medical conditions, leading interdisciplinary care, and helping patients recover, stabilize, and avoid unnecessary hospital readmissions. Key Responsibilities

Serve as the primary physician for patients in skilled nursing and rehabilitation facilities

Manage medically complex post‑acute and long‑term care patients, including multi‑morbidity and polypharmacy

Conduct scheduled and responsive rounds to assess recovery, respond to acute changes, and adjust care plans

Collaborate closely with nursing, rehabilitation, case management, and social work teams

Communicate with patients, families, and caregivers regarding goals of care and treatment plans

Partner with hospitalists, primary care physicians, and specialists to ensure smooth transitions of care

Participate in quality improvement, population health, and value‑based care initiatives

Maintain timely and accurate documentation in the electronic medical record

Qualifications

Medical degree (MD or DO) from an accredited school of medicine.

Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).

Active and unrestricted medical license in California.

Demonstrated experience in post‑acute, skilled nursing, long‑term care, or geriatric settings.

Strong clinical skills in managing complex, multi‑morbid patients.

Excellent communication and interpersonal skills, with a patient‑ and team‑centered approach.

Ability to work effectively in an interdisciplinary, facility‑based environment.

Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications

Fellowship training in Geriatric Medicine or experience in post‑acute/rehabilitation medicine.

Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.

Experience participating in value‑based care or population health programs.
