Care Manager Physical Therapy Home Health

Cheer Home Care is hiring a PT Care Manager in San Diego. The PT Care Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and other healthcare providers. They work closely with our office team and have excellent communication and problem‑solving skills.

The PT Care Manager delivers interventions such as individual treatment plans focused on improving mobility, strength, and independence.

Qualifications

Active California Physical Therapist License

3 years of experience as a Physical Therapist

Home Health experience preferred

Proof of Negative TB result

Physical Screening Documentation within the last year

Valid California driver’s license and car insurance

Willing to work one weekend a month as back‑up on‑call

Ability to multitask, drive, and take action

Strong organizational & communication skills

Cell phone with internet access
