A Brief Overview Provides personal care for elderly, convalescent or disabled persons in home or facility settings. Demonstrates the knowledge and skills necessary to provide care for the population served by Avera.
What You Will Do
Provides assistance to patients with their activities of daily living.
Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care.
Provides services such as housekeeping, laundry, meal preparation, and shopping.
Assists patient with social activities as prescribed by the plan of care.
Maintains timely and accurate documentation.
Reports changes in patient status to supervisor.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served in the agency.
Uses equipment and supplies effectively and efficiently.
Adheres to environmental, safety and infection policies and procedures.
Essential Qualifications
Drivers License - Licensing Board Upon Hire
Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire
Preferred Education, License/Certification, or Work Experience:
High School or GED Equivalent
Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
Less than 1 year in home health care or related health care field
