Nursing Assistant

Hill View is recognized as one of the top nursing homes in Ohio.
Primarily responsible for caring for residents in the Health Care Unit under the direction of nursing staff.
Provides assistance to residents with personal care, including bathing/showers, dressing, undressing, shaving, oral hygiene, skin/hair
ail care, and AM and PM care.
Assists residents with ambulation, toileting, and other activities of daily living.
Answers call lights as needed to determine resident's needs.
Follows each resident's Plan of Care.
Conducts rounds to assigned residents as needed, but not less than once every 2 hours to address toileting and fluid administration needs.
Assures that fresh cold water and a clean glass are always available within reach of the residents. Reminds or assists residents to take fluids during rounds or as ordered.
Immediately reports any unusual observations such as bruises, skin tears or reddened areas, falls, changes from usual behavior, or physical abilities to a nurse.
Takes and records temperature, pulse and respiration rates, blood pressure, food and fluid intake and output as directed. Immediately reports abnormal findings to a nurse.
Serves and collects food trays. Feeds or assists residents who require help eating. Assists residents in completing menus.
Assists with restorative nursing procedures such as body alignment, bed positioning, range of motion exercises, transferring residents, etc.
Follows bowel and bladder training program established for each resident.
Uses Point Click Care system for documentation of behavior and other information as needed.
Assists or gives bathes/showers.
Obtains resident weights as assigned.
Organizes, transports, and assists residents to/with activities and therapy programs.
Assists with resident admissions including:
- greets resident and family member.
- orients resident to room number and location, call light system, and layout of room and facility.
- obtains vital signs, height, weight and documents these.
- unpacks resident clothing and personal items. Assures that clothing and personal possessions are labeled and documented on personal property inventory sheet.
- assures that resident has needed personal care items.
Understands the general use and cleaning procedures of equipment such as oxygen concentrators. Assures that signs are posted on the door of any room where oxygen is being used. Notify nurse if humidifier needs re-filled. Store oxygen cannula properly when not in use.
Follows established infection control policies/procedures. Uses universal precautions when carrying out job duties.
Makes resident beds. Changes bed linens at bath time and as needed.
Assists residents in 24-hour reality orientation.
Runs errands, directs visitors and answers telephones as needed.
Follows safety policies/procedures for preventing, identifying, correcting and reporting hazards and incidents.
Reads department communication book and posted communication board daily.
Is knowledgeable about disaster policies/procedures. Participates in disaster preparedness drills. Takes an active role when a declared disaster occurs.
Is knowledgeable about Wander-guard resident security system. Participates in implementation of the system, in accordance to established policies/procedures. Promptly responds to security alarms.
Completes Behavior ADL monitoring sheet each shift.
Complies with all company policies/procedures related to resident privacy/ confidentiality and information system security, to insure compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Observes facility policies and procedures, including Resident Rights policies.
Performs job duties in an efficient, productive, and professional manner.
Actively supports the implementation of Hill View's mission and statement of purpose.
Participates in training, workshops, etc. to keep abreast of current issues/trends related to his/her job duties. Attends at least 12 hours of in-services per year.
Accepts job assignments with positive spoken word and body language.
Performs other duties as assigned.
This job has no supervisory responsibilities.
Demonstrates good interpersonal and communication skills. The employee must be able to organize work activities with minimal supervision. Must be able to set priorities and plan workload independently.
Must have completed at least two years of high school. High school diploma or GED certificate preferred. Must have successfully completed state-approved Nursing Assistant training program. Must pass state registry test within four months of employment. Must complete 12 hours of in-service education annually. Long Term Care experience.
