About the job LNA
Responsibilities:
Obtains and records Vital Signs and weights per policy;
Participates in the care planning process and implements care according to care plan;
Provide hands on patient/client care;
Grooming of patient/client;
Uses Standard precautions when caring for all patients. Adheres to contact and or droplet precautions as indicated;
Feeds or assists patients with meals and provides additional nourishment and hydration per care plan;
Emotional support for patient/client;
Participates in quality improvement activities as requested;
Collects and bags soiled linen and delivers to dirty linen area;
Performs skin care treatments, as permitted by state regulation;
Assists physician and or licensed nurse with treatments and procedures as needed;
Collaborates and coordinates with other departments to provide timely effective care consistent with individuals needs, choices and preferences;
Cares for personal belongings;
Reports changes in patients condition, patient/family concerns or complaints to charge nurse and or supervisor;
Toileting and incontinence care for patient/client.
Requirements:
Reliable transportation;
TB shot;
Active NH LNA license;
Have reliable transportation and meet Agency requirements for vehicle insurance;
Physical exam within the last year;
Must be licensed or license eligible in VT and NH;
Automobile insurance
