Overview

HealthPartners is looking for an exceptional, patient focused Advanced Practice Provider to join our team and support clinic visits for the New Richmond, WI and Stillwater, MN locations, featuring digestive care/gastroenterology practices.
GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this field. They provide new patient assessment for liver and general GI disease; ongoing chronic disease management; and between-visit partnership with patients in disease management. They are an integral part of the GI team and support HealthPartners patient population with GI and liver disease.

Required Qualifications

Successful completion of an approved Nurse Practitioner program or accredited Physician Assistant program.
Licensed as a Physician Assistant OR currently licensed as an APRN in the state of Wisconsin & Minnesota and current Wisconsin & Minnesota RN license.
Current APP certification as Physician’s Assistant OR current APRN Specialty Certification.
For PA, must be approved by Wisconsin & Minnesota Board of Medical Practice for prescription writing privileges.
Current BLS
Current DEA certification
Must maintain CME requirements
1-year experience as a NP or PA

Position Summary

Regions Digestive Care is staffed by Advanced Practice Providers dedicated to clinic and hospital service, alongside GI physicians with varied clinic rotations and schedules. GI Advanced Practice Providers manage a full patient caseload and provide new patient assessments, chronic disease management, and between-visit partnership in disease management. They are integral to the GI team and essential to patient care.
Major Duties and Tasks

Percentage of time and representative duties are as follows:
Obtains history and performs examinations; documents in the patient record (20%).
Orders appropriate labs and follow-up care; documents orders and notifies personnel (10%).
Instructs patients and families regarding treatment and follow-up; documents patient education (20%).
Communicates and follows up with patients between visits; reviews labs/tests and may include scheduled telephone visits (10%).
Interacts with other healthcare professionals; obtain information, consults, provide feedback or instructions (10%).
Completes required paperwork timely; maintains and reviews patient records and posts results (10%).
Collaborates and coordinates with other health professionals; referrals, consultations and care coordination (5%).
Coordinates flow of inpatient service; prioritize consultations and coordinate with inpatient APP and rounding physician partner (5%).
Discusses and develops diagnosis and treatment plans with staff gastroenterologist; orders diagnostics and documents in EMR (5%).
Discusses with and instructs patient/family regarding treatment and follow-up; documents education and prescribes medications as indicated (5%).
