Job Summary: We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant to join an Immediate Care Center in a vibrant regional area of the Midwest. Candidates with prior urgent care, emergency room, or family practice experience with a strong clinical background are highly preferred. This role involves providing comprehensive assessment, evaluation, planning, and management of patient care for acute and chronic illnesses or injuries under physician supervision.

Work Schedule: The position requires 72 hours bi-weekly, including 1.5 weekends every 4 weeks and 3 holidays per year on a rotating holiday block. The center operates Monday through Friday from 8:00 AM to 7:30 PM, and weekends from 8:30 AM to 4:00 PM, with shortened hours on certain holidays.

Essential Responsibilities:
  • Operate autonomously using critical thinking and problem-solving skills to evaluate family practice patients in an onsite clinic setting.
  • Conduct detailed patient evaluations, interpret lab results, radiographic images, and medical records to arrive at accurate diagnoses.
  • Initiate appropriate orders and document patient history, physical exams, assessments, and care plans thoroughly in medical records.
  • Collaborate with physicians, medical assistants, and other healthcare professionals to coordinate patient care and wellness coaching.
  • Provide individualized clinical education on managing conditions such as hypertension, diabetes, and depression.
  • Refer patients to wellness coordinators for additional support in areas like activity, stress management, and nutrition.
  • Prepare discharge paperwork, including instructions and prescriptions.
  • Order diagnostic tests and referrals as needed for comprehensive patient care.
  • Communicate effectively with patients, families, referring physicians, and clinic staff to ensure seamless care delivery.
  • Participate in quality improvement, safety initiatives, and utilization reviews to enhance patient outcomes and cost-effectiveness.
  • Alert physicians promptly to critical changes in patient status or abnormal test results.
  • Adhere to medical staff policies and maintain accurate, complete medical records.
Education and Experience:
  • Completion or near completion (within 4 months) of a graduate program accredited by a recognized accrediting body, or
  • Completion of a certificate program with national certification requiring a certifying exam.
  • Board certification must be obtained and maintained within two years of program completion.
Licensure:
  • Current, unrestricted Registered Nurse license to practice in the relevant state.

Work Shift: Day and evening shifts available.
