Market Quality Coordinator
The Market Quality Coordinator is responsible for the monitoring and pursuit of CMS STARS HEDIS measures, conducting patient outreach calls, and partnering with center leadership and teams to achieve STARS and HEDIS goals. This role serves as part of the market operations team, working across many payors within a single market in an outpatient senior-focused primary care setting.
Duties/Responsibilities
Conduct enterprise and local strategies and initiatives for closing STARS gaps
Medical record review to document and close Focus On Care measures
Request medical records and upload documentation to electronic medical records system
Perform patient outreach calls to support quality gap closure, including assisting and encouraging members with scheduling and completing screenings.
Schedule and perform ancillary visits to complete in-center screenings.
Meet regularly with Market Leaders to discuss roadblocks, regular progress, and to strategize center and market focused initiatives.
Ensure compliance with OSHA safety standards and conduct accurate, legal, and ethical documentation at all times.
Additional responsibilities and duties as needed.
Required Skills/Abilities
Working knowledge of quality measures and quality gap closure strategies.
Experience working in a primary care setting.
Proficient with Microsoft Office Suite products including Word, Excel, Power Point, Outlook, and Teams
Demonstrated team building skills.
Ability to work independently with a high level of attention to detail.
Sensitivity to the needs of culturally diverse staff, patients, and communities.
Maintains HIPAA standards and confidentiality of protected health information; reports critical incidents immediately.

Education and Experience
High school diploma or equivalent education (GED) required.
A minimum of 1 year work experience as a Medical Assistant
Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e, foreign-trained physician, paramedic, etc.) required
BLS for Healthcare Providers required
