Physical Therapist (PT) – Weekends – Boston Area
All Care is seeking a weekend Physical Therapist (PT) in the Boston area. Our VNA Physical Therapists help patients recover at home, reducing hospital stays and supporting healing in a familiar environment. They create and deliver personalized, one-on-one rehabilitation programs for patients recovering from surgery, illness, or injury, while monitoring progress through regular, in-home care.

Schedule: Saturday and Sunday, 8 a.m. – 8 p.m. No holiday rotation required.

Work Location: On the road in assigned territory, including Boston, Cambridge, Somerville, and surrounding areas.

Position Type: Full Time

Practice independently in the community as part of an interdisciplinary care team.

Conduct comprehensive assessment of patients by determining patient needs, developing courses of action including treatment plan and outcomes.

Manage assigned caseload; provide physical therapy services to patients as directed per PT plan of care.

Deliver personalized rehabilitation therapy to patients in their home or care facilities.

Constantly evaluate evolving patient needs and respond with plan of care adjustments.

Document all care provided and maintain records per All Care policies and standards.

Participate in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.

Travel to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.

Assist patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals.

Use and assist patients with durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.

Qualifications

Valid driver’s license is required.

Active Massachusetts Physical Therapist (PT) license is required.

Previous experience as a physical therapist, preferably in a home health care or outpatient setting.

Experience working with patients recovering from surgery, strokes, or managing chronic conditions is an advantage.

Strong knowledge of physical therapy techniques, modalities, and exercise prescription.

Excellent communication and interpersonal skills to effectively educate and support patients and their families.

Ability to work independently and manage patient care in a home setting.

Proficiency in using electronic health record (EHR) systems for documentation and communication.

Problem‑solving skills to modify treatment plans based on patient progress and challenges.
