Field Nurse Case Manager
Job Description

Home-based Field Nurse Case Manager with travel up to 2 hours one-way (4 hours round trip per appointment). May have multiple appointments per day.
Responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. Works with insurance carriers, medical care providers, attorneys, employers, and employees. Closely monitors progress and reports results to employers and insurance carriers.
Responsibilities

Perform comprehensive assessment of injured workers.
Develop and implement individualized case management plans.
Coordinate care with medical providers, attorneys, employers, and insurance carriers.
Monitor progress and adjust plans as needed.
Report outcomes and documentation to stakeholders.
Qualifications

Current, unencumbered Registered Nurse (RN) license in California.
Experience in case management of workers’ compensation cases preferred.
Preferred certifications: CCM, CRRN, CDMS, CRC—required within two years of hire.
Strong organizational, timeline adherence, PC & technology, written, verbal, and interpersonal communication skills.
Ability to operate autonomously with minimal oversight.
Skill in patient education.
Valid driver’s license with good driving history.
