
IV Pharmacist

Actalent is searching for an IV Compounding Pharmacist – Long-Term Care (Closed-Door Pharmacy).
Key Responsibilities
Perform sterile IV compounding in accordance with USP standards and company policies
Evaluate IV equipment, supplies, and medication compatibility
Prepare and oversee Total Parenteral Nutrition (TPN) formulations
Conduct high-quality clinical checks for medication interactions, incompatibilities, and adverse reactions
Review and evaluate medication orders for accuracy, appropriateness, and therapeutic effectiveness
Ensure all dispensed medications are accurately prepared, supplied, and properly labeled
Provide evidence-based recommendations for medication therapy and dosing adjustments
Supervise and verify the preparation of medications by pharmacy technicians
Maintain accurate and up-to-date patient and customer records within pharmacy systems
Communicate effectively with long-term care facility nursing staff, prescribers, and other healthcare team members regarding medication-related issues
Support regulatory compliance, quality assurance, and continuous improvement initiatives
Qualifications
PharmD or Bachelor of Science in Pharmacy from an accredited school of pharmacy
Active, unrestricted pharmacist license in the state of practice
Previous experience with IV compounding and sterile preparations required
TPN experience strongly preferred
Long-term care or closed-door pharmacy experience preferred
Strong knowledge of medication compatibility, stability, and IV therapy standards
High attention to detail with a commitment to quality and patient safety
Excellent communication and collaboration skills
Ability to supervise and mentor pharmacy technicians effectively
Work Environment
Closed-door pharmacy setting (no direct patient retail interaction)
Focused, production-based IV compounding environment
