Job Overview
As a Home Care RN, you will provide care to patients in both a home setting and independent living facilities. Your daily responsibilities will include collaborating with the caregiving team, patients, and families to provide the highest level of care.

Job Details
Competitive pay, up to $42/hour available
Participate in administrative responsibilities, including documentation and communication with the office team, with paid admin time provided
Flexible full‑time and part‑time hours available – build your own schedule
Weekends not required
Evaluate clients and develop care plans for clinical and non‑clinical teams to follow
Provide training and education to patients, families, and care teams to ensure safety in the home and promote independence
Work alongside the care team to develop care plans and complete documentation
Assist with promoting client independence and healing
Demonstrate competency in carrying out patient care/treatments
Requirements
Must have an active RN license in the state of New Hampshire
LPNs are also welcome to apply, but must possess an active license in New Hampshire
1 to 2 years prior experience
Active CPR certification
Valid driver’s license and reliable transportation
Clean driving record and background check
Per state regulations COVID with booster and flu vaccinations mandatory unless medical exemption presented
