Occupational Therapist (OT) – Boston Area
All Care is seeking an Occupational Therapist to provide skilled therapy services to patients in their homes to improve independence, safety, and quality of life. This role focuses on helping patients regain or maintain the ability to perform daily activities following illness, injury, or functional decline.
Schedule: Monday-Friday
Work Location: On the road in assigned territory, including Boston and surrounding communities
Position Type : Full Time
Responsibilities
Perform comprehensive in-home evaluations of patients’ functional status and living environment
Develop and implement individualized treatment plans to improve activities of daily living (ADLs)
Provide therapeutic interventions to enhance strength, coordination, mobility, and cognitive function
Assess home safety and recommend modifications or adaptive equipment
Educate patients and caregivers on exercises, safety techniques, and use of assistive devices
Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care
Monitor patient progress and adjust treatment plans as needed
Complete timely and accurate documentation in the electronic medical record (EMR)
Ensure compliance with regulatory and agency standards
Qualifications
Valid driver’s license is required.
Active Massachusetts Occupational Therapist (OT) license is required
Previous experience as an occupational therapist, preferably in a home health care or outpatient setting.
Problem-solving skills to modify treatment plans based on patient progress and challenges.
Excellent communication and interpersonal skills to effectively educate and support patients and their families.
Ability to work independently and manage patient care in a home setting.
Proficiency in using electronic health record (EHR) systems for documentation and communication.
