Responsibilities:

Provide skilled occupational therapy in patient homes, evaluate functional status, develop individualized care plans, and collaborate with the healthcare team to support independence and safety. The caseload is 5–6 visits per day; documentation is completed using Homecare Home Base EMR. Mileage is reimbursed at $0.72 per mile.

Qualifications

Current Delaware OT license.

Home health experience.

One reference covering entire work history.
