
Inpatient Pharmacist (Columbia County)

Job Summary
The Pharmacist is responsible for appropriate, evidence‑based medication use for the institution, ensuring patients receive the correct medication at the correct dose. Pharmacists oversee pharmacy technician activities, engage in teaching, and maintain compliance with all applicable standards and regulations (TJC, BOP, CMS, etc.).
Core Responsibilities and Essential Functions
Provide clinical pharmaceutical care to patients, preventing, detecting, and resolving medication‑related problems and making evidence‑based, patient‑centered medication therapy recommendations.
Participate as an effective member of interdisciplinary care team(s) and communicate follow‑up with the team; effectively verify patient medication orders for appropriateness and accuracy.
Provide medication education to patients, families, and team members; enhance the quality of care delivery by actively participating in performance/quality improvement and patient safety initiatives, including medication event/adverse reaction reporting, clinical guideline development, disease management, medication use evaluation/improvement, and cost‑containment actions.
Represent pharmacy services on institutional/practice area committees and quality unit councils; conduct monthly unit inspections of assigned areas.
Demonstrate proficiency in pharmacy distributive services, ensuring quality, safety, and efficiency in the distribution process; collaborate with pharmacy coworkers to ensure effective communication, customer service, and safe drug distribution.
Provide oversight for pharmacy technicians and participate in opportunities to improve the quality, effectiveness, and efficiency of medication distribution.
Support the education and professional development of the Pharmacy Department and other hospital disciplines (nursing, medical, etc.); precept residents, pharmacy students, or pharmacy technicians as assigned; participate in development of new programs, services, and practices.
Engage in professional self‑development by completing competency requirements, establishing regular meetings with management, and attending seminars, continuing education programs, and department meetings.
Demonstrate active membership, service, or leadership in professional organizations.
Perform other duties as assigned.
Comply with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education
Bachelor's degree in Pharmacy or Doctorate in Pharmacy (preferred).
Required Minimum License(s) and Certification(s)
All certifications are required upon hire unless otherwise stated.
License:
RPH – Registered Pharmacist.
Required Minimum Experience
Minimum two years in pharmacy as a technician, intern, and/or pharmacist.
