Job Description
Provides optimal pharmaceutical care for patients through dispensing prescribed medications, monitoring patient medication therapies, providing consultation and education to patients about medications and their use. Acts as the medication subject matter expert as part of the interdisciplinary care team. May advise physicians and other health practitioners on selection, dosage, interactions, and side effects of medications. Provides onsite oversight of day-to-day operations, supporting other staff pharmacists, pharmacy technicians, and pharmacy students or interns.

Principle Responsibilities

Reviews medications

Reviews patient medication profile for possible interactions.

Evaluates medication orders for appropriate dose, route, frequency, and cost-effective administration.

Checks for medication over/under utilization.

Monitors prescriptions to assure effectiveness and appropriateness.

May monitor ongoing therapy evaluating results. May conduct medication history reviews.

Dispenses medications

Enter, verify, compound and dispense medications and IVs.

Verifies accuracy of product preparation, compounding, and labeling.

May evaluate compatibilities between medications, IV solutions, and parenteral nutrition.

May program and maintain pump technology and medication delivery devices.

May reduce audit risks through proper checks such as DAW, days supply, signatures, dates, take as directed, and returns-to-stock to ensure all legal and privacy requirements are met.

May engage in inventory management processes to optimize utilization of ordering and return programs.

May contribute to the team by ensuring medications dispensed to and used by patients are safe, effective, cost effective and meet evidence-based guidelines.

Provides counseling and consultation

Counsels patients on new medication, refills and over-the-counter products as needed.

Consults with physicians and other health care providers to assist patient with medications.

Provides pharmacotherapeutic recommendations to optimize therapy and benefit both patient care and cost efficiency.

Provides medication education and patient care services

Provides medication education to patients to improve adherence and support positive outcomes.

Provides medication information and education to healthcare professionals.

Makes recommendations to optimize medication therapy.

May present on therapeutic topics to discuss prevalent or problematic issues related to prescribing.

Holds self and team members accountable to provide exemplary service to patients, care givers, nursing and other professional staff.

Collaborates as part of Interdisciplinary Care Team-based approach to patient care with physicians, other providers, nurses, and other health professionals.

Performs Lead Duties

May perform Pharmacist In Charge (PIC) duties.

May assist in ensuring that monthly inspections of medication storage areas are conducted and verified.

Assists in providing supervision of daily operations which may include preparation and sterilization of parenteral drugs; bulk compounding of pharmaceuticals; establishing specifications for procurement of drugs and chemicals; developing and maintaining the pharmacy information systems medication list and/or the hospital formulary system; reviewing and updating all medication pump libraries; maintaining stock of antidotes and emergency drugs; maintaining pharmaceutical service records; and providing work direction for pharmacy staff.

May complete and submit to the Board of Pharmacy any necessary licensure applications and/or variance renewals.

Participates in establishment of policies and procedures.

Participates in planning and implementing workplace environmental (space, floor planning) changes.

Participates in the review process along with development and application of pharmacy staff competency validation.

Organizes and participates in precepting, orientation and training new employees, students, interns, and residents.

Identifies and leads process improvements in methods and workflow used in the pharmacy.

Engages pharmacy staff to share direction from leadership ensuring understanding of core direction, roles and responsibilities.

Ensures pharmacy environment meets standards for cleanliness, safety and presents a professional atmosphere.

Other duties as assigned.

Required Qualifications

Bachelor's degree in Pharmacy

Doctoral/Ph.D. level advanced degree in Doctor of Pharmacy (PharmD)

Completion of an accredited residency program

0 to 2 years identified pharmacy experience

2+ years home infusion pharmacy experience including therapeutic drug monitoring and IV admixture, and medication compounding

Preferred Qualifications

Pharmacist board certification if employed in a metro setting

Completion of an accredited residency program

Pharmacist board certification if employed in a community setting

2+ years hospital pharmacy experience

2+ years of retail pharmacy experience

Licenses/Certifications

Licensed Pharmacist - MN Board of Pharmacy required upon hire if working in MN

Licensed Pharmacist - WI Dept of Safety & Professional Services required upon hire if working in WI

Pharmacist Board Certification preferred

Physical Demands

Medium Work: Lifting weight up to 50 lbs. occasionally, up to 25 lbs. frequently

Pay Range
Pay Range: $68.73 to $95.09 per hour. The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
