Department Overview
The Digital Health Navigator, working as a Community Health Worker, works in partnership with teams, staff and clinics across OHSU Health to contribute to programs that aim to improve access to digital health for Oregonians and SW Washingtonians. There are currently three focal points for this role.
1. Digital health navigation support for patients
Provides culturally appropriate digital health navigation support directly to/with OHSU Health patients, building patient comfort, confidence and skills with digital health technology. Focuses on supporting patients in completing virtual/video outpatient/ambulatory visits through phone-based support, using a relationship-building and coaching-first approach.
2. State-wide capacity building through Oregon ECHO Network
Contributes subject matter expertise to develop curriculum, participates in training sessions, delivers components of sessions when needed, and provides technical assistance for participating community-based organizations, supporting a Community of Practice on a monthly basis.
3. Health education and promotion
Develops and delivers culturally congruent and trauma-informed health education classes, presentations and written materials for a variety of audiences; develops systems and services encouraging patient engagement and behavior change, including community-based interventions, home visits and family system interventions.
The role is grant funded for approximately 16-19 months, with grant funding ending October 31, 2027.
Function/Duties of Position
Direct Patient Services and Documentation and Program Development
Respond to requests for support with digital health navigation from care team members and/or self-referrals.
Deliver culturally appropriate digital health navigation support directly to patients.
Build patient comfort, confidence and skills with digital health technology through a relationship‑building and coaching‑first approach.
Assist patients in completing virtual/video outpatient/ambulatory visits through phone‑based support.
Call patients without a referral to offer support.
Apply knowledge of digital health tools and usage across diverse populations, including mother/baby, substance use, mental health, senior services, disability, basic needs, chronic disease management, supportive counseling, etc.
Document services appropriately in the Electronic Health Record and other locations, in accordance with HIPAA guidelines and workflow standards.
Maintain availability via department standard means (phone, pager, email, etc.).
Maintain professional boundaries as part of the care team.
Collaborate with the digital health navigation team to enhance and adjust direct patient services.
Oregon ECHO Training and Capacity‑Building
Contribute to curriculum development as a consultative subject matter expert.
Participate in planning meetings, program sessions (trainings) for Community Based Organizations (CBOs), and deliver components of trainings when applicable.
Provide technical assistance for participating CBOs post‑training, facilitating an ongoing Community of Practice.
Health Education/Promotion
Develop and deliver culturally congruent and trauma‑informed health education classes, presentations, and written materials for healthcare providers, community members, and patients.
Create systems and services that encourage patient engagement and increase health behavior change, including community‑based interventions, home visits and family system interventions.
Assessment for Spanish Language Proficiency
In order to communicate with patients in Spanish, the person hired for this role must pass a speaking and listening assessment administered by Alta, assessing ability to communicate in Spanish in a non‑medical setting. Candidates who indicate they meet the preferred qualification for Spanish skills will be asked to share details of their Spanish‑speaking and writing abilities during the review process.
Required Qualifications
High school diploma or equivalent.
At least 2 years of relevant work experience in a mission‑driven organization, with at least one year working with underserved populations having complex medical and social needs. Preferred experience in community outreach services providing health information, advocacy, social support, and assistance engaging with the healthcare system.
Certification as a Traditional Healthcare Worker (THW) required (Peer Support Specialist, Peer Wellness Specialist, Personal Health Navigator, or Community Health Worker). Type preference may be determined by service need.
Fluency in written and spoken English, with excellent telephone communication skills.
Valid motor vehicle driver license in good standing, or ability to access transportation to meet position requirements.
Basic Life Support (BLS) from AHA required; new hires will complete BLS during orientation.
Job Related Knowledge, Skills and Abilities (Competencies)
Intermediate proficiency in Microsoft Word, Excel, and Outlook.
Comfort using websites, mobile apps, and other computer and smartphone‑based platforms, and interest in helping others learn how to use them.

Preferred Qualifications
Bachelor’s degree.
One year of experience providing direct services to underserved populations with complex medical and social needs.
Experience working in a medical service delivery system or similar setting.
Experience delivering trainings, health promotion/education, and/or providing technical assistance.
Experience with an electronic health record; EPIC platform preferred. Interest and experience in program development, improvement, and assessment.
Spanish‑speaking language skills meeting OHSU's requirements for patient verbal and written communications in Spanish.

Frequent interruptions in an environment of frequent change and fluctuations.
Potential exposure to body fluids, tissue, possible radiation exposure, and infectious processes.
Subject to computer work for several hours, as needed.
Ability to utilize office equipment including computer, smartphone, and pager at workspace and in community.
While the majority of work is anticipated to be provided from a remote or non‑OHSU location, the role must have the ability to provide services and/or attend in‑person meetings in the Portland metropolitan region, perhaps daily as the position evolves.
