ADVANCED PRACTICE PROVIDER (APP) - Advanced Practice Nurse or Physician Assistant – Family Medicine Open Access – Cherry Hill, NJ
Assumes responsibility as an APP practicing within a clinical specialty. All duties and responsibilities must be accomplished in accordance with Cooper Hospital policy and procedures.
The Advanced Practice Provider (APP) in ambulatory office working in open access scheduling should demonstrate clinical expertise in assessment of patients with various medical complaints. The APP must have diagnostic reasoning skills and the ability to therapeutically manage a variety of medical illnesses and injury in a timely manner. This position will include independent evaluation and formation of treatment plans by the advanced practice provider. Consultation by the attending physician will always be available and will be utilized at the discretion of the advanced practice nurse.
Responsibilities
Perform a comprehensive history and physical exam of patients with chronic, urgent and emergent conditions
Document the patient encounter in a thorough manner in the electronic health record
Formulate a differential diagnosis and treatment plan for patients presenting to the ambulatory office
Order and interpret diagnostic tests, including diagnostic x-rays and basic lab tests including but not limited to, urinalysis, CBC and basic chemistry
Assess the severity of illness in a particular patient, and seek consultation and guidance from the attending physician if the complexity of patient’s condition warrants physician involvement or is beyond the demonstrated scope of practice of the individual APP
Communicate pertinent patient information to the collaborating/consulting physician when appropriate
Provide patients with discharge instruction and teaching with each encounter and direct patient to appropriate follow-up care
Perform patient follow-up calls per department protocols
Experience Required
A minimum of 2 years in Urgent Care or Primary Care is preferred but not required.
Education Requirements
Master's degree in Nurse Practitioner or Physician Assistant
License/Certification Requirements
Board certification Nurse Practitioner or Physician Assistant; Active New Jersey License; American Heart Association BLS and CDS/DEA. Certain assignments/locations may require advance certification (ACLS/PALS)
Special Requirements
Scheduled Days / Hours: Monday – Friday; Full time 40 hours weekly
