Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. JOB SUMMARY/OVERVIEW:
The IBH Provider, is responsible for providing quality behavioral healthcare services to patients including psychosocial assessment, brief intervention, referral, brief warm handoff and consultation to medical providers and documentation completed within 48 hours. This position may be a combination of remote and on-site work.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
Education: Master’s in social work, marriage and family therapy, and/or counseling from an accredited school required. Current license or associate certificate required.
Experience/Lived Experience: Experience working in an integrated behavioral health setting providing brief-solution focused services strongly preferred. Knowledge of the current Diagnostic Statistical Manual of Mental Disorders and ability to accurately use this knowledge to diagnose patients/clients and inform treatment planning strongly preferred. Knowledge of psychotropic medications and physical health conditions/chronic disease preferred. Knowledge of crisis intervention, applicable laws and ethical consideration related to patient rights, treatment, safety and confidentiality required. Experience providing therapeutic services to people within a wide range of age/life circumstance and behavioral health history required. Experience working with diverse populations displaying respect and sensitivity to diversity, equity and inclusion preferred.
Special Skills/Training: Strong verbal and written communication skills. Bilingual (English/Spanish) language preferred. Strong interpersonal and customer service skills required; ability to handle difficult patient situations. Strong computer skills, electronic health record system preferred. Must have strong analytical and problem-solving skills. Must certify and remain current in CPR.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Prioritizes, schedules and conducts brief evidence-based therapy for patients/clients with mild to moderate behavioral health symptoms and health behavior issues
Assesses, refers and advocates when appropriate, for services outside of the brief therapy model of care
Provides crisis intervention and brief warm handoffs as needed
Provides education and support to patients and their families to encourage compliance with medical and behavioral regimes
Communicates and collaborates with appropriate community partners
Provides consultation to appropriate primary care staff and providers on behavioral health issues of patients and guide and assist in making CPS/APS reports or contacting law enforcement as needed
Documents patient visits and related information into the electronic medical record in a timely (within 48 hours) and thorough manner.
Develops and presents trainings, in-services, and updates to providers and staff on behavioral health topic
Writes letters documenting and/or summarizing diagnoses and treatment as requested and when ethically and legally appropriate
Attends and presents at group and individual supervision, in a frequency prescribed by the Director of Integrated Behavioral Health or per the BBS guidelines.
Presents and reviews consent for treatment for behavioral health therapeutic services or MAT services. Provides BH support for the MAT program.
Maintain professional license or associate certificate as per California Board of Behavioral Sciences
