ACE Nursing Model of Care: Achieving Collaborative Excellence
Geisinger is piloting a new model of care on our Advanced Acute Care Units at our Danville, Bloomsburg, and Shamokin campuses. The ACE Nursing Model of Care utilizes more Patient Care Technician staff to perform duties that do not require a nursing license, enabling more opportunities for career advancement and greater involvement in patient care.
Primary duties may include:
Being the primary facilitator of ADLs: hygiene, toileting, mobility, meals
Collecting vital signs and daily weights
Conducting point of care testing
Turning and repositioning patients
Performing equipment checks and exchanges
Dressing care
Removal of Foley Catheter and Peripheral IV
Nasogastric tube removal and nasal swabs
Checking code carts and oxygen storage cabinets
Job Description:
May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintain the patient care environment consistent with policies and procedures. Meets the qualifications to provide care for patients in the age range for their assigned clinical area.
Responsibilities
Provide enhanced patient care under the direction of the licensed nurse, including advanced dressing care, suctioning, and removal of Foley Catheter and peripheral IV
May perform venipunctures and skin punctures to obtain blood specimens according to CLSI guidelines
May assist with storage, preparation and delivery of breast milk, infant and pediatric formulas
Document in the medical record; observe and document patient needs and report changes to licensed personnel promptly
Obtain and maintain specimen integrity per policy and procedure; coordinate transport to the laboratory
Assist with activities of daily living and ongoing continuum of care; assist with hygiene and linen changes
Provide one-to-one patient safety watch as assigned
Assist with ordering supplies and basic clerical tasks related to admissions, transfers, discharges, and deaths
Coordinate transportation of patients for tests and coordinate electronic information relevant to the unit
Assist with transporting medications to and from Pharmacy and Blood Bank as needed
Support discharge processes and participate in regulatory readiness as needed
Schedule and Location
GP3 Advanced Acute Care Medical Unit
Schedule: Three 12-hour rotation shifts (7:00am–7:30pm and 11:00am–11:30pm), including every other weekend and holiday. Orientation: two-week classroom orientation at start of employment (7:00am–3:30pm, Monday–Friday).
Requirements
Education: High School Diploma or Equivalent (GED) – Required
Experience: Minimum of 1 year related work experience – Required
