As a Bluegrass Care Navigators' Hospice Team member, you will be part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve comfort and quality of life. Our service allows our team members to focus on one patient at a time, providing care at the patient’s home, nursing homes, assisted living facilities, hospitals, or one of our Hospice Care Centers when needed.

Position Summary
The Certified Nurse Assistant (CNA) is an essential member of the hospice care team who works under the supervisor of a Registered Nurse (RN). This role provides personal care and support services to meet the patient’s daily needs, enhance comfort, and maintain a safe environment. The CNA is also responsible for observing the patient’s condition and accurately reporting and documenting observations and services provided.
Position Qualifications
At least 18 years of age and a high school graduate or equivalent.
Currently registered as a State Nurse Aide with the Kentucky Board of Nursing.
Completion of a recognized Certified Nurse Assistant (CNA) training program.
One year of experience in a hospital or nursing home setting, preferred.
Ability to read, understand, and follow written instructions and accurately document care provided.
Self-directed with the ability to work independently with minimal supervision.
Demonstrates empathy for individuals who are ill, frail, or impaired, and understands the philosophy of hospice care and the needs of the terminally ill.
Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
