Overview
FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Fillmore and the surrounding areas.

ONE-ON-ONE TREATMENT : Observe, record, and report the patient\'s response to treatment and changes to the patient\'s condition. You determine the plan of care.
EASY DOCUMENTATION : Our Electronic Medical Record system is intuitive, making documentation straightforward. Many clinicians complete documentation during the visit.

STREAMLINED REFERRAL PROCESS : Access patients quickly using our app Cliniconnects to accept or decline patients in seconds.
Responsibilities
Deliver quality home healthcare to patients of all ages in the assigned service area.
Observe, record, and report patient responses to treatment and changes in condition. Help determine the plan of care.
Provide one-on-one treatment and maintain accurate, timely documentation.
Coordinate with administrative support for scheduling, care coordination, and quality assurance.
Qualifications
Ability and enthusiasm in working with all ages
Current Physical Therapist license issued by the state Board of Physical Therapy
License must be current, active, and unrestricted; a conditional, provisional, or restricted license will not be accepted. Limited permits will not be accepted.
Completion of an accredited Physical Therapy program
Reliable personal transportation
Smartphone
Bilingual a plus
