SUMMARY OF POSITION:
We are seeking an experienced Advanced Practice Provider (NP or PA) to join our outpatient Endocrinology team with a focus on diabetes management. This role is dedicated to providing comprehensive care for patients with Type 1, Type 2, and complex diabetes conditions, supporting optimal glycemic control and overall health.
Key Responsibilities
Perform detailed assessments and develop individualized diabetes care plans.
Initiate and adjust insulin therapy, oral medications, and advanced diabetes technologies (CGM, insulin pumps).
Provide patient education on nutrition, lifestyle, and self-management strategies.
Collaborate with endocrinologists, dietitians, and diabetes educators to ensure coordinated care.
Monitor and manage acute and chronic complications related to diabetes.
Qualifications
Experience in diabetes management or endocrinology strongly preferred.
Familiarity with insulin pumps, CGM devices, and diabetes education best practices.
MINIMUM QUALIFICATION(S):
Masters Degree in Nursing with clinical specialization required.
Two (2) years\' work experience as a RN required.
Current licensed to practice as a CRNP by Pennsylvania Board of Nurse Examiners required.
PREFERRED QUALIFICATION(S):
Certification as an NP in the area of practice specialization is preferred
