Social Worker

Highland Care Center, a 320 bed Skilled Nursing Facility in Queens, is seeking a Full Time Social Worker. The candidate shoud have experience in a long- term care/sub-acute setting.
Duties include the following:
Interview resident/families for compilation of a social history and completion of a pyschosocial assessment.
Develop and maintain care plans based on individual psychosocial needs of residents.
Facilitate and participate in the interdisciplinary care plan meetings.
Strengthen and facilitate communications among residents, families, and staff.
Act as a discharge planning coordinator to ensure safe discharge to the community.
Provide supportive counseling and assistance as clinically indicated.
Requirements:
B.A. in Social Work or related field.
MDS experience preferred
NYS Screen certification preferred
Experience with EMR (Point Click Care) is a plus
Excellent writtnen and communication skills.
