Position Overview
Under the direct supervision of Program Supervisor and overall direction of the Program Manager, the Social Worker II provides highly advanced, senior‑level social services and case management. Duties include case management for Children and Pregnant Women (CPW), Newborn Screening (NBS), Childhood Lead Poisoning Prevention, Personal Care Services (PCS), Children’s Health Insurance Program (CHIP), Child Protective Services (CPS), and family case management under the Children with Special Health Care Services Program (CHSCN‑SP). The role involves home visits, intakes, family needs assessments, service plans, follow‑up services, referral to community resources, and comprehensive eligibility assessments.

Core Responsibilities

Maintain case files and accurately complete intake, family needs assessments, service plans, and follow‑up documentation.

Link families to community resources and assist with eligibility determinations.

Conduct home visits and presentations to consumers and the public.

Participate in Community Resource Coordination Groups, case‑management coalitions, and community organizations.

Enter and maintain data in reporting systems such as SHSS Task Reporting Qualtrics and CMIS database.

Complete administrative duties, special assignments, monthly reports, and work activities professionally and timely.

Provide 24/7 emergency support when on rotation and assist during regional or state disasters.

Maintain confidentiality and meet all data‑entry deadlines.

Essential Job Functions
Client Services (40%)

Maintain case files and provide specialist social work consultation and assessment to determine PCS eligibility.

Conduct needs assessments, create individualized service plans, and perform follow‑up visits; document information in online database.

Community & Outreach (25%)

Collaborate with community groups, public health entities, and regional staff.

Attend community events and provide presentations and information to stakeholders.

Promote efforts to service families with children who have health risks and special health care needs.

Reporting (15%)

Report all SHSS activities to meet regional program goals and objectives.

Document case‑related and community activities in the appropriate database within required time frames.

Quality Assurance (15%)

Complete corrective actions recommended by QA audits and maintain updated documents.

Document corrective actions via CMIS communication log.

Assist in mentoring new staff and conduct peer reviews.

Adherence to Policy & Other Duties (5%)

Support disaster response and/or continuity of operations as needed.

Perform programmatic processes for upcoming events and maintain a valid Texas class C driver’s license.

Maintain knowledge of HR, IT, administrative policy, and program policy.

Report time, labor, and travel information promptly.

Licensure and Registration
• Current professional license to practice social work in Texas (LBSW, LMSW or LCSW).
• Current license to practice nursing in Texas (optional).
• Valid Class C Texas driver’s license or equivalent.

Knowledge, Skills, Abilities
Knowledge:

Case management principles, Medicaid, CSHCN, CPS, SSI, NBS, lead prevention, ECI, CHIP, quality assurance processes, community resources, cultural/socioeconomic issues.

Skills:

Use of word processing, spreadsheets, databases, email, MS Office; compiling reports; conducting assessments and service plans; providing presentations and training; conflict resolution; teamwork; interpreting policies; professional leadership; confidentiality; time management; transport availability; independent judgment; adherence to DSHS policies.

Abilities:

Provide training; communicate effectively; maintain professional standards; follow procedures; plan orderly caseloads; build consensus.

Initial Screening Criteria

Evidence of at least 2 years in case management principles.

Experience with local resources (2 years).

Organizational skill to meet deadlines (2 years).

Computer proficiency – Office Suite, spreadsheets, databases, presentation software (required – 2 years).

Oral and written communication (preferred – 2 years).

Salary follows DSHS starting salary guidelines and is contingent upon available budgeted funds.
