We are seeking a caring, skilled, and dependable Registered Nurse (RN) to join our clinical team and make a meaningful difference in the lives of patients and families.
Position Summary
The Registered Nurse is responsible for providing skilled nursing care to hospice patients in their homes or place of residence. This role coordinates patient care with the interdisciplinary team, physicians, families, and referral sources to ensure quality, compassionate care throughout the hospice journey.
Essential Responsibilities
Perform comprehensive patient assessments to identify physical, emotional, psychosocial, and environmental needs
Develop, implement, and update individualized plans of care
Provide direct nursing care and symptom management
Coordinate services with physicians, aides, LVNs, social workers, chaplains, and other team members
Educate patients and families regarding care needs and disease progression
Ensure accurate and timely clinical documentation
Maintain compliance with hospice regulations and payer requirements
Participate in on-call rotation as assigned
Promote HOST values, standards, and quality patient care
Other duties as assigned
Qualifications
Education:
Graduate of an accredited Diploma, Associate, or Bachelor School of Nursing
Licensure:
Current Texas RN license
Current Texas Driver's License
Experience:
Minimum two years RN experience in a clinical setting preferred
Hospice experience strongly preferred
Skills:
Strong clinical nursing judgment
Excellent communication and interpersonal skills
Compassionate bedside manner
Ability to work independently and collaboratively
Requirements:
Reliable transportation
Current auto liability insurance
Ability to travel locally throughout service area
CPR certification
Ability to lift up to 50 lbs. and assist with patient mobility
Ability to work flexible schedule and occasional on-call needs
