Visiting Health Services of New Jersey (VHSNJ), a partnership between Hackensack Meridian Health and St. Joseph's Health, provides a comprehensive continuum of home health care and hospice services, support, and resources to patients and families in Passaic, Bergen, and parts of Morris counties. The VHSNJ multidisciplinary clinical and support teams provide high quality care to patients where they most want to be—in their homes.

Responsibilities

Provide in‑home, para‑professional services necessary for caring for patients' personal needs under the direction of the RN.

Perform or assist patients with personal care activities such as baths, showers, skin and hair care, and oral hygiene; assure universal safety precautions and care‑plan direction are followed.

Notify the RN of any changes in patient condition observed in a timely manner and document appropriately; record all required information on the patient chart accurately per organizational protocol.

Assist patients with transfer and ambulation; perform range‑of‑motion exercises; follow safe body‑mechanics principles.

Perform general housekeeping of the patient's area, change patient bed linens, and assist family with light housekeeping such as dusting, vacuuming, and damp mopping.

Assist patients with self‑administration of oral medications, remind them when to take them.

Prepare simple meals in accordance with dietary instructions and assist patients with eating if necessary.

Assist with Foley catheter and colostomy care to ensure proper hygiene; change colostomy bags, perform peri‑care, measure urine, and assure skin is clean around the stoma.

Other duties and/or projects as assigned; demonstrate flexibility with job responsibilities in all areas.

Adhere to HMH's organizational competencies and standards of behavior.

Lifting: minimum 50 lbs.; pushing and pulling minimum 50 lbs.; standing minimum 8 hours a day.

Qualifications

Good customer service skills.

Preferred Qualifications

Home care experience.

Licenses & Certifications

Homemaker – HHA certification.

Valid driver’s license from a U.S. state.

Compensation & Benefits
HMH is committed to pay equity and transparency for our team members. The posted rate of pay is a reasonable, good‑faith estimate of the minimum base pay for this role and does not reflect the full value of our market‑competitive total‑rewards package. The starting rate of pay is provided for informational purposes only and is not a guarantee of a specific offer. Minimum rate of $19.33 hourly.

In addition to compensation for full‑time and part‑time (20+ hrs/week) positions, HMH offers a comprehensive benefits package that includes health, dental, vision, paid leave, tuition reimbursement, and retirement benefits. Performance‑based incentives, bonuses, commissions, and shift differentials may be available for suitable positions.
