Salus Hospice is urgently seeking a compassionate and skilled
Hospice Admissions RN
to join our dedicated team serving the
Orange County
region. If you’re passionate about providing exceptional hospice care and want to work in a collaborative, supportive environment, this is the opportunity you've been waiting for!
This position will cover the following areas: All of Orange County with the ability to cover South Bay, LA as needed.
Schedule
This is a WEEKENDS ONLY Position: Scheduled Saturday and Sundays 10 AM - 8:30 PM
Hospice Admissions RN Responsibilities
Participation in the information gathering, analysis, education and form completion aspects of the admissions process
Evaluate patients for hospice appropriateness, e.g. medical records review as requested
Obtain agreement for admission of referred patients and families who meet admission criteria
Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus Hospice
Maintain clinical and professional competence
Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care
Hospice Admissions RN Qualifications
Registered Nurse with valid CA state license
Minimum one year of hospice or home health experience
Strong background in medical/surgical, ICU/CCU, or ER is helpful
Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residence
Must have excellent interpersonal skills and a passion for hospice
Understanding of hospice philosophy and needs of the terminally ill
Self-directed with ability to work with little supervision
Current Physical and TB test results
