POSITION SUMMARY
The Team Lead Care Coordinator provides direct Care Coordination services while serving as a point person for complex cases, mentorship, and departmental support. This licensed clinical role maintains a caseload but also assists in managing high-acuity patients, placement barriers, length of stay challenges, and departmental workflow needs. The Team Lead supports the Manager of Care Coordination in daily operations and helps foster a culture of collaboration, accountability, and excellence in patient-centered care.

POSITION QUALIFICATIONS
Minimum Education
– Bachelor of Social Work or Nursing
Minimum Experience
– 5 years nursing or medical social work
Required Registration/License/Certification
– Active Alabama license by State Board of Social Work or Registered Nurse
Preferred Education
– MSW or MSN
Preferred Experience
– 8 years of nursing or social work experience in case management or medical social work
Preferred Registration/License/Certification
Case Management Certification from an Accredited Organization
Licensed Independent Clinical Social Worker (LICSW)
