We’re seeking a Social Worker (LCSW)

This position plays a key role in advancing our Housing First approach by supporting guests and residents through assessment, counseling, crisis intervention, and coordination of care. You’ll work closely with our team and community partners to help individuals navigate challenges, access services, and move toward long-term stability. This is hands‑on, relationship‑based work that requires flexibility, empathy, and sound clinical judgment.
Role Details

Onsite, full‑time (40 hr/week), Exempt
Salary: $70k
Schedule: M-F, 8am-4:30pm
Primary Responsibilities

Conduct psychosocial assessments and develop practical, person‑centered service plans
Provide supportive counseling and crisis intervention when needed
Help coordinate care with hospitals, providers, and community partners
Support individuals through transitions, including hospital admissions and discharges
Accompany guests and residents to appointments and advocate on their behalf
Facilitate groups based on program needs
Maintain clear, timely documentation to support care and program compliance
Collaborate with staff across programs and serve as a mental health resource
Participate in the on‑call rotation
Qualifications

Master’s Degree in Social Work (or related field), LCSW required
At least 2 years of experience working with individuals living with mental illness and experience working with individuals experiencing homelessness
Ability to build trust and work effectively with a diverse and often complex population
Strong communication, collaboration, and problem‑solving skills
Valid NYS Driver’s License and ability to transport clients as needed
A belief in early intervention, harm reduction, and meeting people where they are
