Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin's Assisted Living community (Gurwin Jewish : Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies. Our innovative facility and growing services offer opportunities to advance your career. We are currently seeking an Administrative Clerk for our Home Care Agency.
Education and Experience Requirements
College degree preferred.
Minimum of 2 years of home care or related office experience.
Ability to type 50 wpm and good computer skills a must.
Excellent interpersonal and organizational skills required.
Job Responsibilities
Answer telephone for LHCSA and CHHA, record and relay all messages, prepare call slips when indicated, and manage on‑call scheduling with the answering service.
Effectively communicate with patients, family members, physicians, discharge planners from hospitals and staff.
Check eligibility of potential CHHA patients on various electronic platforms.
Enter new and maintain patient demographics and insurance information in the Crescendo (AlayaCare) system.
Upload and file patient referral documents, clinicals, MARS, order summaries, progress notes, H&P, medication lists, hospital discharge summaries, and face‑to‑face notes.
Run and send monthly census analysis to management and intake team by checking eligibility platform.
Actively monitor the WellSky and Navi‑Health platforms throughout the day for potential clients.
Assist in marketing efforts for potential clients.
Demonstrate an attitude of cooperation and professionalism when dealing with peers and supervisors, performing work in the true spirit of the team.
Perform job functions with maximum efficiency and effectiveness, consistently aware of the fiscal impact of actions.
Successfully meet targeted performance indicators related to the clerical position and perform other tasks as assigned.
Accept direction and constructive feedback, comprehend overall concepts communicated, and make necessary changes to improve results.
Consistently perform responsibilities with a focus on customer service, understand internal and external customers, address issues, and resolve them through to full recovery.
Maintain appropriate level of confidentiality consistent with HIPAA regulations, adhere to compliance policy and procedures, and represent the agency in a professional and courteous manner.
Perform related tasks as assigned.
