Position:

Medical Social Worker (Masters Degree Required)

NHC HomeCare Cookeville is looking for a Medical Social Worker to join our team. The Medical Social Worker supervises and provides evaluation of the social and emotional needs of the patient related to their medical condition, and provides counseling based on this evaluation.

Qualifications
All states:

Master’s Degree from a school of social work accredited by the Council on Social Work Education.

One year social work experience in a health care setting is required.

PLUS state-specific requirements:

Tennessee specific:

a. Current state certification as a certified master social worker (CMSW as provided in T.C.A. Section 63-23-102.), or

b. Current license as Licensed Clinical Social Worker (LCSW)

Position Highlights

Provides and documents direct social services to patients planned to restore the patient to optimum social and health adjustment.

Assesses social and emotional factors to minimize impact on the patient’s health and ability to cope with problems of daily living.

Helps patients and caregivers understand, accept, and follow medical recommendations.

Assists patients and caregivers with personal and environmental difficulties that predispose toward illness or interfere with obtaining maximum benefits from medical care.

Identifies resources such as caregivers and community agencies to assist the patient to resume life in the community, including discharge planning.

Assists the providers and home care team members in understanding significant social and emotional factors related to health problems.

Participates in the development of the plan of care.
