Admissions Coordinator
Full-Time | Day Shift
We are seeking an organized and professional Admissions Coordinator to support the admissions process from referral through resident arrival. This role is critical to ensuring admissions are medically appropriate, financially viable, and completed in compliance with regulatory and facility requirements.
This position is ideal for someone who is detail-oriented, personable, and comfortable working with residents, families, hospitals, and internal teams.
Responsibilities
Coordinate the admissions process from initial referral through resident admission
Review referrals to ensure medical appropriateness and insurance viability
Communicate with hospitals, discharge planners, residents, and families
Complete and verify all admissions paperwork and required documentation
Explain facility services, payer sources, and financial information to residents and families
Coordinate with Nursing, Social Services, and Business Office to support smooth admissions
Schedule and conduct facility tours as needed
Update census boards and admission tracking tools daily
Assist with discharge planning and follow-up as assigned
Maintain strong working relationships with referral sources and community partners
Participate in surveys, audits, and admissions-related quality activities
Maintain confidentiality and comply with HIPAA requirements
Qualifications
High school diploma required; bachelor’s degree preferred
Prior admissions or healthcare experience preferred
Long-term care or hospital discharge planning experience strongly preferred
Knowledge of Medicare, Medicaid, and payer sources preferred
Strong communication, organization, and follow-up skills
Professional, dependable, and resident-focused
Physical Requirements
Office and facility-based work throughout the day
Standing, walking, and touring the facility as needed
Frequent interaction with residents, families, and staff
