Description of Work
Reporting to an Aging Care Management Supervisor, the Community Health Nurse will provide clinical consultation to care managers on client cases, conduct in‑home nursing assessment and evaluation of older adults in the community, and deliver training on relevant health and medical topics to older adults, staff, and community partners as needed. The position offers a blend of office‑based responsibilities and fieldwork.
Work Schedule and Additional Information
Full‑time employment
Work hours are 8:30 AM to 4:30 PM, Monday–Friday, with a 30‑minute lunch
Telework: Opportunity to work from home part‑time after six months, depending on operational needs. Must have a securely configured high‑speed internet connection and work from an approved location in Pennsylvania. If unable to telework, the option to report to the headquarters office in Eddystone is available.
Salary: In some cases, the starting salary may be non‑negotiable.
Required Experience, Training & Eligibility
Qualifications
Two years of community health nursing experience and a bachelor’s degree in nursing, including the required number of credit hours of community health nursing education accredited by the National League for Nursing (a master’s degree in nursing or public health accredited by the National League for Nursing or the American Public Health Association may substitute for one year of community nursing experience; or any equivalent combination of experience and training).
Necessary Special Requirement
Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non‑renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.
Conditions of Employment
Employees possessing non‑renewable temporary practice permits must obtain licensure as a Registered Nurse within one (1) year period as defined by the Pennsylvania State Board of Nursing.
Other Requirements

You must be able to perform essential job functions.
Legal Requirements
This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance.
