Hospice Lpn Opportunity With Journeycare Hospice

Journeycare Hospice is seeking a hospice LPN to join our growing crisis care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Hours: Monday - Friday (3) 12-hour shifts 7pm-7am and every other weekend

Territory: Northbrook, IL

Salary: $26 - $38 hourly - offer based on years of experience

Qualifications:

  • Graduate of an accredited practical nurse or a vocational nursing program
  • Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
  • Currently licensed as an LPN/LVN in the practicing state
  • Must possess current CPR certification
  • Demonstrates good verbal and written communication, and organization skills
  • Valid driver's license with an automobile that is insured in accordance with state requirements
  • Understands the philosophy of hospice concept and the needs of the terminally ill

What you'll do:

  • Provide direct patient care as defined in state nurse practice act
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
  • Provide appropriate pain/symptom management. Evaluates the patient's response to treatments/medications
