Key Responsibilities
Monitor and assess patient health data collected through remote monitoring devices
Communicate with patients and their healthcare team to provide updates on patient progress and address any concerns or issues
Collaborate with other healthcare professionals to develop and implement individualized care plans for patients
Educate patients and their families on how to use remote monitoring devices and manage their health conditions
Maintain accurate and up-to-date patient records and documentation
Stay current on industry developments and best practices related to remote patient monitoring
Participate in regular team meetings and training sessions to enhance skills and knowledge
Qualifications
Active nursing license in the state of Idaho
Minimum of 2 years of experience in a clinical setting, preferably in home health or remote patient monitoring
Strong understanding of chronic disease management and remote patient monitoring technology
Excellent communication and interpersonal skills
Ability to work independently and remotely
Comfortable with technology and able to quickly learn new systems and software
Bilingual (English/Spanish) skills are a plus
