In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Seattle/

Job Highlights
The visit lasts about 45 minutes and allows time to connect one‑on‑one, answer health questions, review medication and medical history, perform a physical evaluation, and, if ordered by the health plan, conduct additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation. You are not required to prescribe medicine, order lab tests, or alter the patient’s current treatment regimen.

Help bring flexible, comprehensive, and personalized health evaluations right to patients’ front doors.

Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for their unique needs.

Work with Signify Health as a clinician colleague and fulfill the responsibilities of the specified role.

Be flexible to travel locally and within licensed states, as needed.

Conduct virtual visits as needed.

Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams.

As an in‑home clinician, your primary workplace is the private residence of health plan members. Your environment may vary widely and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to perform your duties safely and comfortably in such settings.

Per Diem Employees Qualify For:

Hourly base rate, plus additional incentive structure (e.g., per visit completed).

Malpractice insurance coverage during Signify Health‑related activities.

Supplies and other perks.

Required & Preferred Qualifications (NPs and PAs)

Active, unrestricted license(s) in the coverage area(s).

Board certification (required).

Multi‑state licenses (preferred).

Ability to cover the following primary location: Seattle / Pierce County.

Compensation
The typical pay range for this role is $46.03 to $99.14 per hour. The actual base salary will depend on experience, education, geography, and other relevant factors.
