Position Summary
We have an exciting opportunity to join our team as an LPN – Homecare, Nassau County Visiting Nurse/ Multiple patients a day. In this role the successful candidate plans the delivery of safe, therapeutic, age‑appropriate patient care, collaborates within a multidisciplinary health team, and performs multiple clinical and clerical duties, complying with policies and regulations, and observing HIPPA guidelines.
Job Responsibilities
Perform other duties as assigned.
Complete documentation reflecting multidisciplinary coordination. Document teaching/learning needs of patients and families while on program and in preparation for discharge. Evaluate patients' response to nursing and multidisciplinary team interventions, including patient/family teaching. Resolve patient clinical issues at point of care. Take action to optimize patient outcomes. Participate in identification, investigation and resolution of risk management, quality care and ethical issues. Address patient issues following established communication hierarchy.
Evaluate patient progress toward health goals and modify critical path or plan of care accordingly. Interact and collaborate with other health team members to identify patient needs. Initiate interventions to amend variances from plan of care. Document patient and family responses and collaborate with the RN to modify the plan of care as needed. Provide direct patient care per policies, guidelines and physician orders. Incorporate standards of practice, clinical pathways and age‑specific guidelines into plan of care.
Recognize and respond to issues that may cause problems or refer issues to the RN, using escalation policy. Recognize the family defined by the patient and incorporate principles of family‑centered care. Identify patient satisfaction/dissatisfaction with the plan of care and appropriately resolve. Verify and document patient advance care directives. Maintain clear communication channels through intra‑ and inter‑shift reporting. Use appropriate communication, demonstrate proper telephone etiquette, and participate in maintaining a clean, safe therapeutic environment.
Obtain prior authorization for medication from insurance companies as needed; obtain referrals for patient tests ordered by MD per insurance requirements. Maintain exam room supplies and cleanliness.
Process medication orders and refills through the Workflow system. Verify dosage and medication, enter comments and initials in Workflow for verbal approval from a physician to refill prescriptions.
Identify patient satisfaction/dissatisfaction, apply conflict resolution skills, and address issues with the plan of care. Assess and document patient/family advance care directives and rights and responsibilities if indicated. Establish collaborative relationships with health‑care team members.
Minimum Qualifications
High School or Equivalent Technical Diploma.
Graduate of an accredited School of Licensed Practical Nursing.
0‑1 year nursing experience in a hospital, ambulatory, private practice or home health care setting (preferred).
Required Licenses
LPN.
Qualified candidates must be able to effectively communicate with all levels of the organization.
